Mandatory Forms required for all TKA Athletics Grades 6-12

Forms_image001FHSAA EL2 & FHSAA EL3 must be complete and on file in the Athletic Office prior to try outs/ athletic participation.
These forms can be faxed or copied and sent in to the Athletic Office.
Filling out your email address on the FHSAA EL2 will ensure a confirmed receipt of the Athletic Department receiving the forms.

FHSAA EL2- Preparticipation Physical Evaluation (Required)
*Turn in a new form every school year.

  • Fill out form completely. Signatures are required at the bottom of page 1 from both Parent/Guardian and Student. Physician will fill out and sign page 2 or 3.
  • This form will be valid for 365 calendar days from the date of the evaluation.
    • *We cannot accept Palm Beach County and/or School District Health Forms or Health Exams.


FHSAA EL3- Consent & Release of Liability Certificate (Required)
*Turn in a new form every school year.

  • Fill out form completely. Insurance information is required including Insurance Company name and Policy number. *You must purchase insurance for the season your child plays his/her sport if you don't currently have insurance. Please contact the TKA Business Office for questions regarding supplemental insurance.
  • Student and Parent signatures are required.
  • Certificate is good for current school year only.


TKA Medical Release Form Disclaimer- All students are required to have one medical release form signed, notarized and turned in to the Elementary/Secondary Office. These forms are collected at the beginning of each school year and are valid for the current school year only. 6th grade medical release forms will be kept in the Elementary office and 7th – 12th grade medical release forms will be kept in the Secondary office. The TKA Athletic Department will make a copy of the form once team rosters have been determined. The copies will then be given to the coach for the season.

FHSAA GA4 - Affidavit of Compliance on Recruiting Form (Required for Transfers only)

  • Required for all 9-12th transfer student-athletes. Students who begin their freshman year at TKA are not required to complete an affidavit.
